New Patient Registration

Our practice area covers E18 and IG8. 

If you live within these postal districts and you wish to register with us, we ask for you to come into the surgery to fill in a registration form. We would like all new registrations above the age of five years old to attend a new patient medical carried out by appointment only with the nurse.

Children under the age of eight months will be invited for child health surveillance. 

As a patient you are registered with the surgery, not a particular doctor.

Changes to Personal Details

It is important that we have the correct contact details for you at all times. If you should move house, or change your telephone number, please let reception know.

Also, if you are receiving hospital care, please inform the staff there of any changes to your details.
